On Thursday, November 30, 2006, the Board of Directors for Consolidated Tribal Health Project, Inc. has changed the policy for covered services for the Contract Health Services Program. The following changes are effective as of Monday, December 4, 2006.

Previous policy:

• Age 65 and over - Level III coverage.
• Diabetic patients 64 and under - Prescription coverage at Level III.
• Age 64 and under - Level II coverage. Exception for eyeglasses and hearing aids (per current policy).
• Catastrophic health care incidents above $25,000 will no longer be covered under Contract Health Services.

Current policy:

• CHS eligible patients age 55 and over - Level III coverage.
• Diabetic CHS patients - Level I, II, and III coverage for prescriptions only.
• CHS eligible patients age 54 and under - Level I for prescriptions and referral services.
• Catastrophic health care incidents above $25,000 will not be covered under Contract Health Services.
