New EHR System

CTHP is implementing a new Electronic Health Record system called Athena effective June 1, 2017.

During the months of June and July, you may experience a longer wait time for your medical appointment while this process happens. We sincerely appreciate your patience as we improve the quality of our patient service delivery.

 

Walk In Hours Now Available in Medical

Monday, Tuesday, Thursday and Friday

1:30 to 5:00 p.m.

Walk in hours are available for sudden onset symptoms. Patients will be triaged and served on a first come, first served basis. The first six patients will be served. Whether or not more patients can be accommodated will depend on the needs of the first six patients. Walk in hours are subject to provider availability.

Walk in appointments are not for prescription refills, physicals or wellness visits.

If a patient cannot be seen, the patient will receive a scheduled appointment or be referred to the closest urgent care.

CLINIC HOURS OF CARE (all clinics)

Monday through Friday from 8 to 12; 1 to 5 pm.

After Hours Care

CTHP offers patients a nurse triage line when after-hours assistance is needed.

This service is NOT for medical emergencies, nor does it replace care by your Primary Care Provider.

Dial 707-485-5115 and choose "Option 3".

If you access the After Hours Nurse Triage, call 707-467-5649 the next clinic day to schedule a follow up appointment with your primary care provider.

 

Lab Hours: 8 am until 4 pm. No lab appointments after 4 pm. See Appointments section about scheduling a lab draw.

 

Medical Specialty Services:  Acupuncture and Asian Medicine; Chiropractic; Massage Therapy; Podiatry. Telemedicine for endocrinology, nutrition and psychiatry consults, and retinal screening.

REGISTRATION (new patients)

Call 707-467-5608 to schedule a time to meet with a Patient Resource Advocate to complete the registration packet and receive apply for coverage.

PRC's registration office is in the main lobby of building B.

PRC's main office is located in the lobby in building A.

Flu and pneumoccocal IZs:  schedule a Nurse Only visit.

 

Law draw appointments. Please request a Nurse Only visit when scheduling your lab draw.

For lab draw requests from outside providers, have the provider fax the request to the medical department at 707-485-1445, or you can drop off the lab draw request at medical reception and schedule your Nurse Only visit. Please allow 12 to 24 hours to properly process your request from an outside provider so that everything is ready when you arrive for your visit.

 

Cancelling or Rescheduling Your Appointment

Please call at least 24 hours in advance where possible. This allows us to accommodate other patients. Scheduling and keeping your appointments is critical in helping you to keep your health and wellness on track.

Please arrive at least 15 minutes prior to your appointment, to allow time to sign in, update your information and complete any required paperwork. This will help us keep our appointment schedule on time throughout the day.

 

Transportation Services

Remember to call Transportation Services to cancel or reschedule your pickup, if one was arranged one for your appointment.

PATIENT VOICE

Patient Feedback System (ongoing). Patient comments, compliments, suggestions and complaints are always welcome.

The most effective way to do this is to complete a Patient Feedback Form. The Executive Director, Richard Matens, will handle ALL Patient Feedback Forms in a timely manner.

Depending upon the patient's stated preference, he may call to follow up with the patient, he may meet with them in person, or correspond in writing.

Patients that provide us with feedback have absolutely nothing to worry about in terms of the care they receive at CTHP. CTHP is here to serve our patients without bias. We cannot improve if we are not aware of what we can do to better serve our patients.

 

Patient Feedback Form: download here or pick one up the next time you are at the clinic. They are available in the main lobby literature display holders. Reception staff will also be happy to provide you with a form.

 

Patient Satisfaction Survey (annual). CTHP conducts an annual Patient Satisfaction Survey to patient satisfaction with various aspects of care. The results help CTHP develop an action plan to improve care for our patients.

Results of these surveys are significant, so please complete and submit a survey in June.

ALERTS 

Communicable & Infectious Diseases

DO NOT COME INTO THE CLINIC if you suspect that you, your child or someone you are bringing to the clinic may have a serious communicable or infectious disease such as chicken pox, measles, mumps, tuberculosis (TB), meningitis, whooping cough, Ebola, Zika, West Nile Virus, etc. Click here to view the flyer.

Dental Emergencies

Go to your nearest emergency room outside of dental clinic hours if CTHP has no urgent care appointments available to assist you.

Follow up the next clinic day by contacting Dental Reception at 707-467-5601 to schedule a follow up appointment if needed.
