A referral from your CTHP provider is required to access specialty services, except for Acupuncture.

Cancellations / Rescheduling
Please call at least 24 hours in advance where possible. This allows us to accommodate other patients. Our appointments are booked several weeks and sometimes months ahead, so scheduling and keeping your appointments is critcal in helping you to keep your visits and oral health care treatment on track.

Staff Providers

Amy Redmer, M.D., Medical Director and Staff Physician

Dr. Redmer grew up in the Midwest. She received her undergraduate degree from Northwestern University in Evanston, IL and completed her medical degree from Chicago Medical School with honors. Dr. Redmer began her practice as a Commissioned Officer in the United States Navy. She did her residency in family practices with the U. S. Navy with follow up tours, traveling around the world to Italy, Japan and the Middle East over a fifteen year period. Dr. Redmer comes to CTHP via an inter-agency transfer to Indian Health Service.

Dr. Redmer joined CTHP ’s Medical Department in May 2016 as a Staff Physician and has since stepped up to head the department as the Medical Director.


James Flaherty, M.D., Staff Pediatrician

Dr. Flaherty is Board Certified in Pediatrics, and with prior subspecialty Board Certification in Pediatric Emergency Medicine. He is a Fellow of the American Academy of Pediatrics, and a member of the AAP’s Sections on Emergency Medicine, Injury Prevention and Poisoning, and Community Pediatrics. Dr. Flaherty grew up in San Francisco, and graduated from Georgetown University School of Medicine in 1983. He did five years of training at Children’s Hospital Oakland, including his pediatric internship and residency, chief residency, and fellowship in pediatric emergency medicine.

Ann Luke, F.N.P., Family Nurse Practitioner

Ann Luke comes to CTHP after seven (7) years with Alexander Valley Healthcare in Cloverdale. She has more than 37 years’ experience in the healthcare field, the last 16 of which has been as a Family Nurse Practitioner. Ann’s broad professional background includes extensive work in geriatrics, public health, acute care and hospice. She has experience in Emergency Room Nursing, as well as performing assessments on patients and families who are facing end-of-life situations. Ann received her Bachelors of Science in Nursing from California State University – Dominguez Hills and her Masters of Science in Nursing and Family Nurse Practitioner from California State University – Fresno.


Dr. Angelo Chavez Guerrero, M.D., Staff Physician

Dr. Angelo Chavez Guerrero, a board certified Family Medicine Physician comes to CTHP from Alabama. He is member of the American Academy of Family Physicians and has been practicing Family Medicine and in Urgent Care Facilities for over 15 years in the United States. Dr. Chavez Guerrero received his medical degree from Universidad Catolica de Santiago de Guayaquil in Equador.
